Uses: 

Clomipramine is a tricyclic antidepressant primarily used to treat symptoms of obsessive-compulsive disorder (OCD), including recurrent thoughts or feelings and repetitive actions. It can be used in adults and children aged 10 years and older.

Side Effects:

  • Dizziness, drowsiness, tiredness, or nervousness
  • Tremors, jerking muscle movements
  • Sweating
  • Changes in appetite or weight
  • Urination problems
  • Vision changes
  • Decreased sex drive, impotence, or difficulty having an orgasm

Serious Side Effects:

  • Low sodium levels (e.g., headache, confusion, severe weakness)
  • Vision issues (e.g., blurred vision, tunnel vision, eye pain)
  • Mental changes (e.g., extreme fear, thoughts of self-harm)
  • Urination pain or burning
  • Seizures
  • Serotonin syndrome symptoms (e.g., agitation, hallucinations, fever)

Seek emergency medical help for allergic reactions or severe drug reactions affecting multiple body parts.

Warnings:

  • Do not use clomipramine if you've had a recent heart attack.
  • Avoid using if you've taken MAO inhibitors in the last 14 days due to the risk of dangerous drug interactions.
  • Young individuals may have suicidal thoughts when starting an antidepressant. Close monitoring by healthcare providers and caregivers is necessary.

Precautions:

  • Inform your doctor if you have conditions like liver or kidney disease, bipolar disorder, schizophrenia, heart disease, low blood pressure, narrow-angle glaucoma, or a history of seizures.
  • Be cautious if using other medications that can interact with clomipramine, such as stimulants, opioids, and certain antidepressants.
  • Consult with your doctor if pregnant or breastfeeding, as clomipramine may cause withdrawal symptoms in newborns if taken during late pregnancy.

Dosage:

Adults:

  • Initial: 25 mg once daily at bedtime
  • Maintenance: 100 mg daily
  • Maximum: 250 mg daily
  • Dosage adjustments should be made gradually to maintain the lowest effective dose.

Children (10-17 years):

  • Initial: 25 mg once daily at bedtime
  • Maintenance: Up to 3 mg/kg or 100 mg daily, whichever is smaller
  • Maximum: 3 mg/kg daily or 200 mg daily, whichever is smaller
  • Administer initial doses with meals to reduce gastrointestinal side effects.

Missed Dose:

  • Take the missed dose as soon as possible but skip it if it's almost time for the next dose. Do not double up doses.

Overdose:

  • Seek emergency medical attention immediately. Symptoms may include irregular heartbeats, confusion, severe sweating, extreme drowsiness, seizures, or coma.

Drug Interactions:

  • Avoid alcohol as it can cause dangerous side effects.
  • Clomipramine may interact with other medications that cause drowsiness (e.g., opioids, sleeping pills, muscle relaxers).
  • Inform your doctor of all medications you're taking, including over-the-counter drugs, vitamins, and herbal products, to avoid interactions.

Storage:

  • Store at room temperature, 20° to 25°C (68° to 77°F), away from moisture and heat. Keep the bottle tightly closed when not in use.

Additional Considerations:

  • Consistent monitoring by a healthcare provider is essential, especially during dose adjustments and at the start of treatment.
