Uses:

  • Hypertension: Clonidine lowers blood pressure by decreasing certain chemicals in the blood, allowing blood vessels to relax and the heart to beat more slowly and easily.
  • ADHD: The Kapvay brand (extended-release tablet) and Onyda XR (extended-release oral suspension) are used to treat attention deficit hyperactivity disorder (ADHD) in children 6 years and older.

Warnings:

  • Inform your doctor if you have heart disease, severe coronary artery disease, heart rhythm disorders, low blood pressure, history of heart attack or stroke, kidney disease, or allergies to Catapres TTS patches.
  • Use clonidine exactly as directed by your doctor. Do not use multiple forms of clonidine simultaneously.
  • Clonidine may affect fertility in both men and women.
  • It's not known if clonidine harms an unborn baby, but pregnant women may be tracked in a pregnancy registry to monitor effects on the baby.
  • If breastfeeding, monitor the baby for signs of somnolence, tiredness, rapid breathing, and poor feeding.

Precautions:

  • Do not take clonidine if allergic to it.
  • Consult your doctor if you have conditions like heart disease, severe coronary artery disease, heart rhythm disorders, high or low blood pressure, fainting spells, a history of heart attack or stroke, pheochromocytoma, or kidney disease.
  • Inform your doctor about any other medications you are taking, including over-the-counter medicines, vitamins, and herbal products, to avoid interactions.
  • Catapres is not approved for anyone younger than 18 years old, and Kapvay should not be given to children younger than 6 years old.

Dosage:

Adults (Hypertension):

  • Oral: Initial dose is 0.1 mg twice daily, increasing weekly as needed. Maintenance dose is 0.2 to 0.6 mg daily in divided doses. Maximum dose is 2.4 mg daily.
  • Transdermal Patches: Initial dose is 0.1 mg/24 hr patch applied every 7 days, increasing as needed. Maximum dose is typically two 0.3 mg/24 hr patches applied every 7 days.

Children (ADHD):

  • Kapvay Extended-Release Tablets: Initial dose is 0.1 mg at bedtime for children 6 years and older. Increase by 0.1 mg/day weekly. Maximum dose is 0.4 mg/day in divided doses.
  • Onyda XR Extended-Release Oral Suspension: Initial dose is 0.1 mg once daily at bedtime for children 6 years and older, increasing weekly by 0.1 mg/day. Maximum dose is 0.4 mg once daily.

Administration:

  • Clonidine should be taken in the morning and at bedtime, with the larger dose at bedtime if doses differ.
  • Swallow extended-release tablets whole without crushing or chewing.
  • Do not stop using clonidine suddenly to avoid withdrawal symptoms.
  • Store at room temperature, away from moisture, heat, and light.

Missed Dose:

  • If you miss a dose, take it as soon as you remember. Skip the missed dose if it is almost time for the next dose. Do not take two doses at once.

Overdose:

  • Seek emergency medical help immediately. Symptoms include feeling cold, irritable, weak, drowsy, light-headed, having weak reflexes, pinpoint pupils, slow heartbeats, shallow breathing, or seizures.

What to Avoid:

  • Avoid alcohol as it may increase side effects.
  • Avoid driving or hazardous activities until you know how clonidine affects you, as it may cause dizziness or drowsiness.
  • Avoid overheating or dehydration during exercise and in hot weather.

Side Effects:

Common:

  • Drowsiness, dizziness
  • Feeling tired or irritable
  • Dry mouth
  • Constipation, loss of appetite
  • Sleep problems (insomnia), nightmares

Serious:

  • Severe chest pain, shortness of breath, irregular heartbeats
  • Very slow heart rate
  • Light-headed feeling, like you might pass out

Drug Interactions:

  • Using clonidine with other drowsiness-causing drugs (opioids, sleeping pills, muscle relaxers, anxiety or seizure medications) can worsen drowsiness.
  • Inform your doctor about all other medications, especially heart or blood pressure medications, antidepressants, and any medicine containing clonidine.
